Professional Service Agreement

On the INPUT page of the Professional Service Agreement form application, you will see a choice of   2 payment options:

1. Payment of fee where the individual is responsible for their own expenses. This option will print Form PSA-1 as the output page.



2. Payment of fee with expenses to be reimbursed seperately. This option will print Form PSA-2 as the output page.

    appropriate payment option.
